Your phone itself hasn't been hacked, unless 1 you hacked it yourself by jailbreaking it and then got infected with malware, or 2 someone malicious has had physical access to your phone.
There's no known malware capable of infecting an iPhone if neither of these conditions is met. So, nothing has been stolen from your phone.
Online accounts of any kind are another story entirely, and can easily be hacked, but this would not be related to any mysterious icons showing up on the iOS desktop. For some reason it created the "red letter D" icon on one of my home screens when I downloaded a new unrelated app. I noticed in iTunes that the "red letter D" app had changed to the discover magazine icon. Once I figured this out, I deleted the discover magazine app from the Newstand app and then the "red letter D" app disappeared as well.
